KEY TERM DEFINITIONS
Program and Location-type definitions:
Oncology Rehabilitation Program (“Program”) – a rehabilitation therapist (Physical Therapist, Occupational Therapist, or Speech Language Pathologist) or team of rehabilitation therapists (including PTAs, COTAs, and SLPAs) providing specialized rehabilitation care to patients with a cancer diagnosis. A program may span one or more of the following:
  • Rehabilitation Facility/Clinic/Location (“Rehab Facility”) – a single geographic location offering oncology/cancer rehabilitation services (e.g. a standalone outpatient rehabilitation clinic or a hospital-based outpatient rehabilitation clinic)
  • Hospital Facility (“Hospital”) – a hospital or cancer service line incorporating or closely associated with a Rehabilitation Facility/Clinic/Location
  • Health System – a group of hospitals and/or facilities (usually grouped within a city, county, state, or group of neighboring states)
 
THE PORI CENTER OF EXCELLENCE STANDARDS
These standards have been designed for the sole purpose of offering oncology rehabilitation programs the opportunity to become a PORi Center of Excellence. In this manual, "Standard" is defined as a qualification for accreditation and should not be construed as a standard of care. These standards do not aim to supersede clinical judgment.
The standards in this manual are divided into three sections:
  • Section 1: Bronze Standards
  • Section 2: Silver Standards
  • Section 3: Gold Standards
Each section is subdivided into categories, with each category containing numerically noted standards. Standards are further broken down into subsections* (denoted with lowercase letters).
* Each subsection must be complete in a standard to meet compliance with that standard.
Each standard includes criteria for compliance. While the standards are designed to be as objective as possible, compliance is ultimately determined by the PORi Center of Excellence Surveyor (the PORi Surveyor) in collaboration with the PORi Center of Excellence Compliance Team.
PORi Center of Excellence Best Practice
Within several PORi Center of Excellence Standards, you will encounter a "PORi Center of Excellence Best Practice" box. These are supplemental to the Standard and are not mandatory for meeting the Standard.
PORi Center of Excellence (Bronze)
Bronze PORI Centers of Excellence focus on education, licensing, and a commitment to the PORi system of patient management at the practitioner level. Bronze Centers of Excellence must meet the following standards:

  • Std 3. Educational Requirements - Individual PORi Certification
  • Std 4. Rehab Facility License Requirements
PORi Center of Excellence (Silver)
Silver PORi Centers of Excellence demonstrate internal collaboration among practitioners and office staff to optimize patient management within the rehabilitation setting. Silver PORi Centers of Excellence must meet the following standards:
  • All Bronze-Level Requirements PLUS:
  • Std 5 a-b. Documentation
  • Std 6 a-d. Patient Care
  • Std 7 a-g. New Patient Processing
  • Std 8 a-d. Coordination of Patient Services
  • Std 9. VOBs and Authorizations - Tracking & Management of Authorizations & Benefit Limits
  • Std 10. Service-scape
  • Std 11. Client Perspective Approach to Cancer Care
  • Std 12 a-e. Therapist Perspective Approach to Cancer Care
PORi Center of Excellence (Gold)
Gold PORi Centers of Excellence demonstrate internal cohesiveness and have formed strong collaborative relationships with other providers within the greater cancer care team such as physicians, nurses, hospital administrators, and community support services. Gold PORi Centers of Excellence must meet the following standards:
  • All Bronze-Level and Silver-Level Requirements PLUS:
  • Std 13. PORi Center of Excellence Committee
  • Std 14 a-d. Interdisciplinary Collaboration (Rehabilitation as 4th Pillar of Oncology Care)
  • Std 15. Data Analysis
  • Std 16. Quality Improvement Initiatives
  • Std 17 Patient Resources
  • Std 18. Wellness Promotion
  • Std 19. Community Outreach
PORi Center of Excellence - Hospitals or Health Systems
For health systems or multi-facility groups, once at least 75% of their locations within a geographic or corporate region are PORi Centers of Excellence, the entire system within that geographic or corporate region can be designated a PORi Center of Excellence.
